First Endodontic Visit
In your initial consultation we’ll explain your diagnosis and treatment options. Typically, we can treat you that same day but more complex treatments may require additional appointments.
Please register online and bring this information to your first visit:
- Referral slip and X-rays
- List of medications you are currently taking
- Dental insurance card, if applicable
Dental Anxiety
If you experience moderate to severe dental anxiety, we can prescribe safe medications to help you have a comfortable experience.
X-Rays
You may request that your dentist forward your x-rays to our office. We may take additional films to help us make an accurate diagnosis. (We use digital radiography, which decreases radiation exposure by as much as 90 percent, compared with conventional x-ray film)
Insurance and Financial Options
Several factors determine the fee for endodontic therapy, including which and how many teeth are to be treated. We discuss all fees before beginning treatment.

We are happy to process your insurance claim for you. Please realize that:
- Your insurance company is a contract among you, your employer, and the insurance company. The contract is not with the dentist.
- Not all services are covered benefits in all contracts. Insurance companies select payment levels based on the insurance contract, not on the dental fees of the community, although they refer to them as “usual and customary.”
We are currently contracted providers for the Delta Dental Network, BlueCross Blue Shield, DenteMax and United Concordia. We also accept all other insurances that offer out-of-network benefits. We accept cash, checks, Visa and MasterCard. We offer a 5% discount for full payment with cash, check, debit or credit. We also offer in-house financing at no cost to you through CareCredit®.
You are fully responsible for all fees charged by this office regardless of your insurance coverage. Please bring your insurance information with you to your appointment.
